Depending on the treatment requirement, stockings are available in either round-knit or flat-knit styles. The compression class is determined based on the specific diagnosis and is prescribed by a doctor.
Round-knit compression stockings are used for venous diseases, while flat-knit compression stockings are suitable for lymphatic conditions.
The fundamental difference lies in how the compression stocking is knitted. As the name suggests, the round-knit stocking is knitted "round" with a decreasing pressure gradient, whereas the flat-knit stocking is knitted "flat" or in rows and later sewn together. The stocking gains its anatomical shape and pressure gradient through the increase or decrease of stitches. For this reason, flat-knit stockings are made to measure. Round-knit stockings, however, are available in various standard sizes or, if necessary, made to measure.
Function of Round Knit
In people with venous diseases, the return of oxygen-poor blood to the heart is no longer as it should be. The reason for this is in the veins which are enlarged. As a result, the venous valves no longer close properly and the blood essentially flows back down into the foot and stagnates there. This causes the legs to swell, and the risk of thrombosis increases significantly.
The pressure exerted by the compression stocking from the outside, combined with the muscle tension that compresses the veins, helps support the return of blood towards the heart.
Function of Flat Knit
In people with lymphedema the function of the lymphatic system is impaired, and the lymph fluid can no longer be properly transported. Then it accumulates in the tissue, causing painless swelling.
Here the compression stocking has the task of applying pressure to the tissue, providing counterpressure for the muscles and the oedema. This promotes lymphatic drainage and prevents the accumulation of lymph fluid in the tissue.
In the case of lipoedema, both - a firm circular knitted compression stocking and a flat knitted compression garment - are possible. This depends on the severity of the condition. Lipoedema is a chronic disease characterized by a disorder of fat distribution, usually in the arms and legs, accompanied by severe pain upon touch. A common symptom of lipoedema, like in lymphedema, is swelling due to fluid accumulation in the tissue. The role of compression therapy in lipoedema is the same as in lymphedema, to apply pressure to the tissue and muscles and provide counterpressure. This supports decongestion and the pressure on the muscles promotes muscle activity.
Compression therapy can also help alleviate pain.

Explanation of terms
Lymphedema
Lymphedema occurs when the function of the lymphatic vessel system is impaired, and lymph fluid can no longer be properly transported. As a result, the fluid accumulates in the tissue, causing painless swelling.
PAD
Peripheral arterial disease (PAD) often represents a contraindication for compression stockings, as the compression can lead to a reduction in arterial blood flow.
Varicose Veins
Varicose veins occur when blood pools in the superficial veins of the legs. This happens when the blood flow back to the heart is impaired due to malfunctioning vein valves.
The veins swell and become visible on the surface. The so-called spider veins are a milder form of varicose veins.
Lipoedema
Lipoedema is a chronic condition characterized by a disorder of fat distribution, usually affecting the arms and legs, and is accompanied by severe pain upon touch. Similar to lymphedema, one of the accompanying symptoms of lipoedema is swelling caused by fluid accumulation in the tissue.
CVI
Chronic venous insufficiency (CVI) is a condition affecting the leg veins where the blood flow back to the heart is impaired. Compression therapy plays a very important role in its management.
Thrombosis
Thrombosis occurs when a blood clot forms in a blood vessel. These clots often develop when the blood flow back to the heart through the veins is no longer functioning properly. Blood pools in the veins, increasing the risk of thrombosis. If the clot breaks loose and travels through the cardiovascular system, it can block vital vessels. Every case of thrombosis is a medical emergency.
